This program assists adults in both Tompkins and Tioga counties diagnosed with behavioral health needs, that is, mental illness and/or substance abuse, to reach greater independence, achieve their life goals and become more involved in their communities.
These Home and Community Based Services (HCBS) can help participants with:
Daily Living and Social Skills:
For example: making social connections, budgeting, advocating for themselves, negotiating relationships
Education and Employment:
Support to start, return to, or graduate from school in order to learn employment skills
Support to define employment goals, find and keep a job, and plan a career
Behavioral Health HCBS are available for people 21 and over who are enrolled in a Medicaid Managed Care and are found eligible after completing the New York State Health and Recovery Plan (HARP) Eligibility Assessment.
